How do I schedule an evaluation?

How long is the evaluation appointment?

Since each evaluation is individualized, the time required will vary from two to six hours. Approximate testing time will be discussed prior to setting up your testing appointment.

What paperwork is needed? 

When you schedule your testing appointment, the diagnostician will send a packet, either digitally or via surface mail, with background information and behavior checklists. When appropriate, teacher forms will also be included in the packet. Please provide your child’s teacher with the forms they need to complete ASAP so they can return their completed forms before testing.

I can’t afford to pay this upfront. Are there payment options?

Yes. Alternate payment arrangements may be made in advance of testing.

Do I need to stay for testing?

You are not required to remain in our office while your child is being tested; however, you are welcome to wait for your child in our reception area. You are free to run errands or work off-site, once your child is settled and as long as they feel comfortable.

What should we bring with us to the testing appointment?

You will not need to bring anything, other than the paperwork we send to you when we set up your child’s evaluation. We provide pencils and all materials needed for testing. We also have water bottles and snacks in our office. If your child has an allergy or food preference, you are welcome to send a snack with them. In the case of a child or adult with severe anxiety, we do offer in-home testing services to enable clients to perform at their most optimal level during the evaluation sessions in a familiar environment.

How do I prepare my child for the evaluation?

Students are often anxious about the testing process. Please explain that they will be doing some work to find out how their unique brain functions and that even though they are called “tests,” there is nothing they can fail. 

I am an adult going in for testing, How do I prepare for the evaluation? 

The same was as if you were a student. Come well rested and prepared to work with us for a time to discover how we can work with you to reach your optimal performance ability. 

Do you test for dyslexia?

Yes, reading is thoroughly covered in our cognitive battery. We do not separate dyslexia testing from the full battery.

At what age can you diagnose dyslexia?

Students need to be in the first grade before a diagnosis can be made. Prior to that time, testing can be used to identify precursors to dyslexia, but more testing is needed to confirm the diagnosis once the student is in the first grade and capable of being assessed in all areas of reading.  The largest body of research reveals full manifestation of dyslexic tendencies by the 2nd grade. 

Do you test for dysgraphia?

Yes, dysgraphia (a writing disorder that impacts the functionality of a student’s writing skills) is incorporated into our full cognitive battery.

Our tutor has suggested our child may qualify for accommodations on the ACT and SAT. Does your testing qualify for documentation?

These institutions typically require two things:
1) Current (within three years) documentation of a disability: the cognitive battery satisfies this requirement.
2) Confirmation that the student has been receiving the accommodation(s) requested in the school setting. The school provides this documentation.

What can I expect after testing?

You can expect answers to the question(s) that prompted the evaluation.  After the testing, you will meet with the diagnostician to review the findings, discuss how the test data supports the diagnosis, and talk about the recommendations based on the evaluation. Parents and/or clients are given a detailed written report. The report also includes relevant recommendations for home and school personnel. 

Our goal is to equip parents with an action plan based on testing, which may include outside resources. We allow enough time during the feedback to review the test results and answer parent questions. Older students are welcome to attend the feedback, or a separate conference can be scheduled for a fee. Feedbacks typically run 1 ½ to two hours.

How do I pay for the evaluation?

Fees may be paid for with cash, check, or credit card (Visa, MasterCard, or Discover). We also accept HSA/FSA (health savings account) payments.  We will provide relevant documentation to support the use of this payment option.  

One half of the testing fee is requested at the time of testing; the balance can be paid at the time of the feedback. Reports cannot be released unless your testing is paid for in full.

Do you take insurance?

No, we are Fee-for-Service providers, and therefore we do not accept insurance and we are not affiliated with any insurance plans. However, your insurance provider may reimburse you for some portion of your testing. If reimbursement is important, you will need to contact your insurance company before you schedule your testing appointment. Please ask if they will cover the cost of a cognitive evaluation. The procedure code is CPT: 96102 (testing conducted by a technician). We will provide a detailed receipt with the CPT and applicable diagnosis codes when your account is paid in full.
